The Universal Service Administrative Company (“USAC”) administers the Lifeline Program which is a federal
program that helps eligible consumers pay for wireless or home telephone service by discounting monthly
service bills. At least once each year, consumers who receive Lifeline Program-supported service must
recertify that 1) they remain eligible, and 2) no one else in their household receives Lifeline Program-supported
service. This website, including its content, systems, and networks, provides information about and
services related to the recertification requirement in the Lifeline Program (“Website”).
This Website is to be used by individuals who (1) are at least 18 years old, (2) are located within
the United States, including its various states and territories (collectively referred to as “United
(4) accept these Terms. If you cannot confirm each of (1) through (4) above, then you may not access
or use the Website.
USAC may amend the Terms from time to time, with or without notice to you. You can see when these Terms
were last revised by referring to the “Last Modified” date on this page. Your continued access to or
use of the Website after USAC has posted revised Terms signifies your acceptance of such revised Terms.
No other amendment to or modification of these Terms will be binding unless in writing and signed by
USAC’s duly authorized representative or posted to the Website by USAC’s duly authorized representative.
USE OF THE WEBSITE
USAC makes the Website available to you for access within the United States solely to enable you to
file your own Lifeline Program Annual Recertification Form electronically with USAC in accordance with
47 C.F.R. § 54.410. (“Authorized Purpose”)
You may use the Website solely for your non-commercial, personal purposes.
Certain features of the Website allow you to post and submit data. You represent and warrant to USAC
that you have all necessary rights, permissions, licenses and/or consents to post and submit the data
The Website may only be used for its Authorized Purpose. This system is monitored, recorded and subject
to audit. Any unauthorized use or misuse of this system is strictly prohibited and subject to legal
action, including criminal prosecution and civil penalties. USAC is not responsible for any costs or
expenses incurred by you as a result of your use of the Website.
Notify USAC if you know or have a reasonable suspicion
that any person or entity has accessed or used the Website other than for the Authorized Purpose.
You waive any claim you may have against USAC relating to its use of the information you provide on
the Website, except to the extent such claim is caused by USAC’s material violation of laws or willful

not using the Website in furtherance of the Authorized Purpose). In using the Website, you may not
(a) falsely state or otherwise misrepresent information; (b) use or provide any fraudulent, misleading
or inaccurate information; (c) defame, abuse, harass, or violate the rights of others, including without
limitation others’ privacy rights or rights of publicity; (d) access or use (or attempt to access or
use) another user’s information; (e) transmit any viruses, worms, trojan horses, malware, or other items
of a destructive nature; (f) “frame” or “mirror” any portion of the Website; (g) use any robot, spider,
site search/retrieval application or other manual or automatic device or process to retrieve, index,
“data mine” or in any way reproduce or circumvent the navigational structure or presentation of the
Website; (h) use the Website for illegal activity; or (i) probe, scan or test the vulnerability of the
Website, breach the security or authentication measures on the Website, or take any action that imposes
an unreasonable or disproportionately large load on the infrastructure of the Website.
TERMINATION OF ACCESS
USAC may, in its sole discretion, terminate or suspend your access and authority to use all or part
of the Website for any reason or no reason, with or without notice to you.
COMPLIANCE WITH LAWS
You will comply with all laws, rules and regulations applicable to your use of the Website.

This Website is protected by United States copyright laws, and belongs to USAC or its partners, affiliates,
contributors or third parties. You may print screens of the Website for your non-commercial, non-public,
personal use only. You are prohibited from using any of the marks or logos appearing throughout the

and all intellectual property or other rights in the foregoing, including but not limited to any and

COLLECTION OF INFORMATION
A. Information You Provide. Personal data is any information that allows you to be identified. USAC
collects personal data you provide through such methods as your submission of forms or questions through
the Website. The types of personal data USAC collects includes your name, address, telephone number,
date of birth, last four digits of your social security number, and Lifeline eligibility information.
Your use of the Website and the actions you take while on the Website represent your consent for USAC
to use the information collected.
B. Information Automatically Collected. When you visit the Website, certain information regarding your
time at the Website is automatically collected. This information includes, but is not limited to your
IP address, browser type, domain names, access times, pages viewed, and referring website addresses.
This information is used to provide general statistics about Website visitors and as otherwise provided
in these Terms.
USE AND SHARING OF INFORMATION
USAC uses information it receives through this Website for purposes of administering universal service
support mechanisms. USAC may disclose information to the Federal Communications Commission pursuant
to 47 C.F.R. Section 54.702(j) or other authorized parties including pursuant to inquiries received
under applicable law. USAC will disclose your information and the status of your recertification to
the carrier who identified you as a subscriber. USAC engages contractors to assist with its administration
of the recertification requirements of the Lifeline Program who will have access to the information
you provide. USAC will also disclose information about you to USAC’s contractors for purposes of administering
the Lifeline Program and/or serving and maintaining the Website.
Engaging in prohibited activity such as unauthorized access to or use of the Website is strictly prohibited
and may result in civil and/or criminal punishment. If attempts are made to obtain unauthorized access
to the system, USAC may provide information to the appropriate law enforcement officials.
Visited pages may be tracked in order to determine which areas of the Website are the most popular.
This data may be used to deliver customized content.
USAC shall use commercially reasonable efforts to secure the information you provide. You must also
take reasonable efforts to secure the information you provide such as logging out of the Website before
leaving it to prevent access to your information from subsequent users of that computer.
